JOB RESPONSIBILITIES

We are looking for a registered nurse for home care in ordinary housing. The job offers the opportunity for long-term patient relationships, independent responsibility, and close collaboration in a team.

As a nurse in home care, you will work with planned, person-centered, and coordinated care for patients in their homes. The work is characterized by continuity, a holistic view, and close cooperation with other professionals. You play a central role in assessing patients' health status, planning and following up on care interventions, and contributing to safety, continuity, and quality for both patients and relatives.

The job responsibilities include, among other things:

• Making medical assessments and planning care and nursing interventions
• Being responsible for medication management, wound care, and other health and medical care interventions
• Working preventively and following up on interventions according to current routines
• Participating in and coordinating care planning, SIP, and follow-ups
• Collaborating with home care, rehabilitation, case managers, and health centers
• Guiding and supporting nursing staff in medical issues

QUALIFICATIONS

Requirements:

• Registered nurse
• Driver's license (B)
• Good ability to work independently and in a structured manner
• Good knowledge of Swedish, both spoken and written

Desirable:

• Experience in municipal health and medical care
• Specialist training in district, geriatrics, or psychiatry
• Experience in home care in ordinary housing
